Floyd locations are currently offering:​​

  • Pfizer vaccine - available to anyone 12 and older. This vaccine requires two doses given three weeks apart. A second-dose appointment will be made at the time the first vaccine is given. Pfizer Vaccine Fact Sheet

Anyone under age 18​ should be accompanied by a legal guardian to provide consent. ​​​​​​

Floyd Primary Care

Floyd Primary Care locations located throughout our community are offering the COVID vaccination to those eligible 12 years of age and older. To schedule an appointment contact your Floyd Pri​mary Care provider​ or call our call center to schedule at a location near you 706.509.3750. ​

Booster Vaccinations​

Floyd is offering eligible, existing primary care patients the Pfizer COVID-19 vaccine booster, in accordance with the CDC's recommendation of booster doses. To be eligible, patients must have received first and second doses of the Pfizer vaccine at least six months prior to receiving the booster. To schedule an appointment contact your Floyd Pri​mary Care provider​ or call our call center to schedule at a location near you 706.509.3750. ​
