Floyd is offering the COVID-19 vaccine​ in line with the Georgia Department of Public Health (DPH) guidelines​​

We are now offering the free vaccine to individuals age 16 and older (depending on vaccine brand). Individuals who live or work in Georgia are eligible as supplies last.  ​

Vaccines are being given at Floyd Medical Center and Polk Medical Center. 


​Make an Appointment

You can make an appointment online for your first dose of the vaccination. Proof of residency and age, such as a driver's license or ID card, will be necessary at the time of your appointment. ​

Floyd is offering only the Moderna and Pfizer-BioNTech​ vaccines, and we cannot accommodate requests for a specific vaccine. Vaccines are administered based on existing supply. ​​
