Primary and Urgent Care
Access your patient health record through the patient portal designed for Floyd primary care and urgent care patients. When logging on to myhealthrecord.com patients can do the following through the secure portal:
- Message your doctor
- Request medication refills
- View prescribed medications
- View visit details
- View labs
Need to Enroll
To enroll, patients must accept a patient portal invitation sent to their personal email address. If you did not provide an email address during your most recent visit or you can't find your email invitation, call 706.509.3236. To receive an invitation, we will ask a series of questions to verify identity. Then an email invitation with a secure, unique link to the patient portal will be sent to email address provided.
Once receiving the secure link, the patient should follow the instructions provided to create a username and password to access the MyFloyd account.
