What is MyFloyd patient portal?

MyFloyd patient ​portal is a secure website that allows read-only access to your personal health information and interact with Floyd family medicine providers. All Floyd patients are encouraged to join and use the patient portal for ready access to your personal health information.​

Why does Floyd have three different portals?

Floyd uses different electronic medical records to provide patient care. Depending on where you receive your health care, you may need access to more than one portal. We thank you for your patience as we work diligently to improve our patients’ experience with this technology and have several improvement measures underway.

The three portals Floyd offers include:

  • Primary Care or Urgent Care - Patients who have visited an Urgent Care facility or a Floyd family medicine provider​ are able to view visit details and  interact with their primary care provider through secure messaging and prescription refill request. 

  • Hospital Care​​ - Patients who have stayed at Floyd Medical Center, Polk Medical Center or Willowbrooke at Floyd are able to view list of medications, diagnoses, allergies, lab test results, patient history and other healthcare information related to your inpatient hospital visit.

  • Floyd Cherokee Medical Center​ - Patients receiving services at the facility located in Cherokee, Alabama can view list of medications, diagnoses, allergies, lab test results, patient history and other healthcare information related to your inpatient hospital visit.

How do I sign up for MyFloyd Patient Portal?

Primary Care or Urgent Care - Access is provided through an email invitation. To receive an email invitation contact a portal service representative at 706.509.3236 or by email to verify your identity and provide an email address. Once the email is sent you will follow the instructions for create a password and login to your account.

Hospital Care - You can self enroll to the hospital care portal if you provided your email address or ​social security number at the time of registration during your visit to Floyd Medical Center, Polk Medical Center or Willowbrooke at Floyd. Access self enrollment.​

Floyd Cherokee Medical Center - Access is provided through an email invitation. To receive an email invitation call 844.686.3498 to verify your identity and provide an email address. Once the email is sent you will follow the instructions for create a password and login to your account.


How can I access the portal after I have an account?

After you have created a patient portal account for one of the portals, use the following to access the portal.

Primary Care or Urgent Care –Access the portal login by going to floyd.org/myfloyd or go to https://myhealthrecord.com/ directly.

Hospital Care – For patients of Floyd Medical Center and Polk Medical Center, you can access the portal at http://floyd.org/myfloyd​ or download the HealtheLife app available for iphone and Android.

Floyd Cherokee Medical Center  - Access the portal login by going to floyd.org/myfloyd or go to https://www.thrivepatientportal.com.
