To help reduce the stress of surgery, we are to assist with your surgery preparation. Below you will find the steps you can take to prepare for your surgery day.
- Pre-surgery Assessment Form
The two forms listed below consists of question about your medical history and general information needed for admission. Patients may complete the Pre-surgery Assessment Form online or at their pre-surgery testing appointment. The Admission Form can be printed, completed and brought to the next visit.
- Pre-surgery Testing
A registered nurse will meet with you to discuss and arrange your pre-admission needs three to five days before your procedure. Patients are encouraged to visit the Outpatient Surgery Center located in the 330 Physician Center anytime Monday through Friday, 8:30 - 2:30 to complete their pre-surgery testing. No appointment is needed.
The pre-surgery testing may include the following:
- Completing necessary forms
- Reviewing your specific medical history, medical issues and needs with a registered nurse
- Completing medical test required for your surgery
- Receiving instructions for the day of your surgery
- Discussing the procedure and aftercare
- Time for you to ask any questions about your surgery procedure
- Receive information to assist you in preparing for your surgery.
Items needed for pre-admission meeting:
- Physician's order and consent forms for your procedure, if not already provided.
- Proof of insurance
- Photo identification
- Medical history, including family medical history
- Admission form
