To help reduce the stress of surgery, we provide assistance with your surgery preparation.Below you will find the steps you can take to prepare for your surgery day.

1. Pre-surgery Assessment Form
   Patients will be asked to complete a questionaire regarding medical history and general information needed for admission. Patients will complete this information at their pre-surgery testing appointment. The Admission Form can be printed, completed and brought to the next visit.

2. Pre-surgery Testing
   Your pre-surgery testing should be completed three to five days before your procedure. For surgery procedures at Floyd Medical Center patients should schedule a pre-admit appointment by calling 706.509.6300.

The pre-surgery testing may include the following:

• Completing necessary forms
• Reviewing your specific medical history, medical issues and needs with a registered nurse
• Completing medical test required for your surgery
• Receiving instructions for the day of your surgery
• Discussing the procedure and aftercare
• Time for you to ask any questions about your surgery procedure
• Receive information to assist you in preparing for your surgery.

Items needed for pre-admission meeting:

• Physician's order and consent forms for your procedure, if not already provided.
• Proof of insurance
• Photo identification
• Medical history, including family medical history
• Admission form
