When it is time for you to be discharged, we want
to make your transition from hospital to home or interim care easy
and convenient.
Before you go
Your physician will let you know when you are ready to
be discharged. Our staff will work to have you ready to
be discharged by 11 a.m. on the day of your dismissal.
- Please ask the person who will be driving you to arrive by 10
a.m.
- If you need help in making transportation plans, your nurse
will be happy to assist you.
- It's a good idea to inspect closets, drawers and the restroom
to make sure you don't leave anything behind.
When you are discharged
Your nurse will give you instructions to follow after your dismissal.
This will include information about:
- Medications
- Diet
- Follow-up appointments
- Home care services
- Special directions or arrangements
This information is important to your recovery. Your nurse will
be able to answer any additional questions and help you make arrangements
for special supplies, home health or hospice services.
